This organization is charged with the development, training and continuing education of pain medicine specialists in Australian and New Zealand. This app was developed by the Faculty of Pain Medicine of the Australian and New Zealand College of Anaesthetists ( FPM ANZCA ). One of the newest opioid conversion apps to enter the fray is Opioid Calculator.

Some apps such as Opioid Risk Tool contained excellent information, but were implemented poorly leading to difficulty just inputting doses of meds.

We assessed output variability between apps in the dose calculation of seven opioid switches, as well as assessing the level of professional medical involvement in the authorship, creation and design of the apps. We searched major online app stores for apps providing an opioid dose conversion tool.

There are significant concerns with regard to the reliability of information provided by apps offering opioid dose conversion, with lack of information regarding evidence-based content and peer review in many cases.

Published data were screened by a Medline search using the keywords: opioid, rotation, switching, potency and dose ratio. A prospective process validation was performed prior to the approval. Five months after the introduction of the tool, an online survey was performed in order to evaluate its acceptance by users. Based on this data, the criteria for the conversion calculator were defined. In the last step, a manual calculation using a written table was compared with the calculator in a cross-over trial with 72 participants in order to assess the findings of the survey.

With this tool we contribute to the optimisation of processes in hospital and patient safety. Based on the results of the online survey and the results of our cross-over trial we conclude that the tool saves time compared to manual calculation and may contribute to patient safety by avoiding calculation errors. A web-based opioid conversion calculator was planned, generated, validated and established at the hospital.

The opioid conversion calculator could be generated for 24 combinations of 6 opioids for 5 different routes of administration. The validation of the calculator was performed successfully without discovering any major or critical defects. The proportion of correct answers increased from 68% using the table to 81% using the calculator (P < 0.001), the median time necessary to answer the ten questions was 8.9 min using the table and 4.8 min using the calculator.

It is designed to provide easy accessibility and consistency to facilitate research, improve education and fill a void in availability of reference conversion data in clinical practice. A particular design feature of the app is that it simplifies the calculation of oMEDD in cases where combinations of opioids are used.
